Supported Independent Living (SIL QDDP)

Summary

Performs and is responsible for the implementation of the various aspects of client programming in the Residential program, including developing, coordinating, monitoring and tracking the effective delivery of services and in assisting the clients to live successfully in their own domicile within the community.  This person is an exempt employee who works whenever needed, including afternoons nights, weekends and holidays.  Responsible for the management and coordination of Residential services to ensure that the delivery of services offered for each of these programs is done effectively, in a timely manner, and according to the Comprehensive Plan of Care and Waiver Regulations.  Responsible for development, implementation, maintenance, and revision of the assigned clients’ Plans of Care in the Residential Program.  Responsible for attending all CPOC and Quarterly meetings for assigned clients in the Residential Program.  Responsible for the training of new hires and preparation of instruction/training information to all PCAs pertaining to the necessary follow through of client plans of care for all assigned clients within the Residential Program.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Provides assistance to clients in all aspects of independent living including acquiring and maintaining food stamps, Social Security benefits, appropriate Section 8 housing, etc.
  • Aids clients in budgeting and paying bills, preparing food, maintaining their home environment, and assisting in the development of the skills to use available transportation.
  • Provides guidance, training and information to improve each individual’s independent living skills.
  • Assists individuals in scheduling and/or attending appointments, making purchases, pursuing recreational activities, money management, obtaining necessary community services and monitoring and placing a reorder with the nurse in a timely manner regarding needed medications, housing and 24 hour safety net.
  • Must have the ability to properly balance checkbooks and create meaningful budgets meeting the needs of the client and balancing his/her money to allow for some recreational opportunities.
  • Acts as a positive role model to clients assigned to caseload.
  • Maintains complete and accurate records, including service logs, progress notes, quarterly summaries, data sheets and financial records.
  • Exercises good judgment in processing irregularities and/or incidents through proper channels in a timely manner and in accordance with all State and Federal guidelines and Waiver Regulations.
  • Develops and maintains a positive relationship with assigned clients and their families through regular personal contacts.
  • Communicates as needed with family members and maintains positive relationships with them.
  • Advocates and promotes each individual’s legal and human rights.
  • Responsible for reporting immediately to Residential Director any problems clients have regarding safety, health and welfare or any other pertinent issues/areas of concern that impact the individual’s served life.
  • Must be available to evacuate with residential clients in the event of an emergency, such as hurricanes. If not needed to evacuate, must be available for coverage and the coordination upon clients’ return.  This includes scheduling client coverage and/or working shifts upon client return from evacuation.
  • Develops, implements and monitors assigned Residential clients’ service plans and progress through weekly contacts, quarterly and annual reviews.
  • Regularly meets with Personal Care Attendants and Support Coordinators to discuss aspects of client care, formulate goals, and address training issues or other matters that directly relate to the clients’ plan and/or care.
  • Gathers, organizes, writes and ensures implementation of assigned Residential clients’ POCs in time frames set forth by OCDD and Waiver guidelines.
  • Provides information, training and assistance to Personal Care Attendants regarding client needs, goals and implementation of service plans.
  • Responsible for ensuring adequate units are in the plan to maintain services and billing.
  • Responsible for contacting proper agencies for revisions needed to plans.
  • Responsible for setting up binders for new clients, filing new/removing old CPOCs and POCs in those binders.
  • Ensures total compliance with Waiver, OCDD, State and all other necessary licensing regulations through appropriate and accurate documentation in a timely manner.
  • Documentation must meet guidelines for OCDD and Waiver regulations and must be turned in to appropriate TARC staff upon request or in compliance with mandated timelines.
  • Ability to work flexible hours and days, including nights, weekends and holidays.
  • Is on-call for assigned Residential clients from 6:00 AM on Monday through 4:00 PM on Friday, rotate on-call weekdays from 4:00 PM-6:00AM and on scheduled on-call weekends. Is to assist other QDDPs and Coordinators with call as needed.
  • Assist fellow QDDPs and Coordinators as needed to complete necessary weekly duties including but not limited to: running appointments, finding staff coverage, working shifts, taking calls, etc.
  • Administer medication to clients as prescribed by physician if nurse delegated; consulting with nurse in emergency medical situations and with questions regarding medication administration; following agency policy and procedure regarding medications.
  • Comply with all TARC’s policies and procedures.
  • Direct supervision of PCA workers assigned.
  • Create staff schedules to cover client needs for assigned caseload
  • Regularly meet with subordinates to discuss aspects of client care, formulate goals, and address training issues or other matters that directly relate to clients.
  • Responsible for the management and coordination of Center-based Respite services in compliance with all regulating agencies.

Supervisory Responsibilities

This position directly supervises PCA workers. Carries out supervisory responsibilities in accordance with the organizations policies and applicable law. Responsibilities include planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Consulting with the residential and/or human resources director as needed for clarification and direction.

Education and/or Experience

Bachelor’s degree (B.S.) in human services field from an accredited college or university with a minimum of one year experience working with individuals with intellectual disabilities or other developmental disabilities.

Certificates, Licenses, Registrations

Current Louisiana Driver’s License

 

Ability to maintain nurse delegation and/or CMA certification

Other Qualifications

Ability to work various hours, including evenings, nights, weekends and holidays.
