Summary:  Performs and is responsible for the implementation of the various aspects of client programming in the Residential program, including developing, supervising, coordinating, monitoring and tracking the effective delivery of services and in assisting the clients to live successfully in their own domicile within the community and community homes.

Responsible for the management and coordination of Residential services to ensure that the delivery of services offered for each of these programs is done effectively, in a timely manner, and according to the Comprehensive Plan of Care and Waiver Regulations.  Responsible for development, implementation, maintenance, and revision of the assigned clients’ Plans of Care in the Residential Program.  Responsible for attending all CPOC and Quarterly meetings for assigned clients in the Residential Program.

Responsible for the training of new hires and preparation of instruction/training information to all PCAs pertaining to the necessary follow through of client plans of care for all assigned clients within the Residential Program.  This person will work when necessary, which includes possible nights, weekends and holidays.

Small Caseload: 16-18

Education: Qualified applicants must have a bachelor’s degree in the Human Services Field and one year of experience working the Intellectual and Developmental Disability population.

Qualified applicants must be able to work flexible hours, have a valid driver’s license and reliable transportation.
