COMMUNITY LIVING SPECIALIST FLOATER-Wellington Home

Summary

Responsible to teach, train, guide and supervise active treatment and all direct care activities with clients in our community homes; maintain the facility and equipment; assure clients rights as per federal regulations; perform the function of a “caregiver” in the absence of the clients family.

Responsibilities

The Community Living Specialist (CLS) is responsible for the direct hands-on teaching and training of clients during active treatment outlined in the ISP.  Provide direct supervision of clients in all aspects of community living.  Completes all written reports concerning client behaviors, incidents, and charting objectives.  Communicates with CHM any information regarding client care, active treatment, etc.  Work is performed in strict confidence and in accordance with agency, state/federal regulations.  Coordination of services and program monitoring will be received from the Community Home Manager and the QDDP.
